Description

The SUD Support Specialist provides support services to, and acts as an advocate for patients of PrimeCare. The SUD Support Specialist works as part of behavioral health team. This role is responsible for supporting behavioral health care within the clinic, specifically focused on substance use disorder (SUD) services, including medication-assisted treatment. The SUD Support Specialist collaborates with behavioral health staff in engaging patients in their own care through assessment, measurement of progress, outreach, education, and referral management efforts. The SUD Support Specialist is responsible for activities related to care management of SUD patients, health insurance enrollment, and prevention education. The position is best suited for energetic, empathetic, and organized individuals, who thrive in a team-based environment.
