Description
The Recovery Resource Specialist will support case management, health insurance enrollment, and an anti-stigma campaign to be delivered throughout the health system. The Recovery Resource Specialist will meet with patients in CCHHS patient care centers working with SUD, including the Central Campus location, Ambulatory Community Health Network (ACHN) sites or Cermak Health Services. The Recovery Resource Specialist will work closely with Recovery Coaches, CADCs, LCSWs, medical providers, and members of the Care Coordination team to identify and link patients to resources to reduce barriers to recovery, including health insurance coverage.
