Description
Under the direct supervision of the Program Manager, the AmeriCorps Residential Level 3.5 Case Manager will be part of the clinical team responsible for managing and monitoring the participants progress within the program; offering technical assistance and intense case management linkage and referrals as needed. The AmeriCorps member will focus on providing case management linkages and referrals to external service providers aimed at minimizing the overall health, legal, and social harms associated with substance use and mental health disorders. Additionally, the member will collaborate with two clinicians, providing clients that have substance use disorders with assessments, treatment plans/case reviews, and working with other community resources in meeting/achieving client service needs. The member will also facilitate group counseling and opportunity to provide individual counseling sessions to participants will/may also be available depending on the member’s readiness and willingness to learn.
