Description

The GRACE (Ground-Level Response and Coordinated Engagement) Program is responsible for providing appropriate and collaborative care for identified clients in Whatcom County. Sea Mar is the contracted provider for intensive case management services through an agreement with Whatcom County. The AfterCare Specialist will provide follow-up care to clients who are using crisis services and law engagement enforcement in frequent, yet ineffective ways. The AmeriCorps member will provided services alongside an advanced registered nurse practitioner (ARNP), Mental Health Specialists, Community Health Worker and an administrative assistant under the supervision of the site supervisor-GRACE Program Manager.
