The GRACE (Ground-Level Response and Coordinated Engagement) Program is responsible for providing appropriate and collaborative care for identified clients in Whatcom County. Sea Mar is the contracted provider for intensive case management services through an agreement with Whatcom County. The AfterCare Specialist will provide follow-up care to clients who are using crisis services and law engagement enforcement in frequent, yet ineffective ways. The AmeriCorps member will provided services alongside an advanced registered nurse practitioner (ARNP), Mental Health Specialists, Community Health Worker and an administrative assistant under the supervision of the site supervisor-GRACE Program Manager.
Service Activities
• Participate in mandatory trainings, service projects and team building activities throughout their 10-11 month full time term of service.
• Provides outreach to community partners and specialists as appropriate to enlist their collaboration in working with the program and enrolled clients.
• Coordinates follow-up care with clients enrolled in the program.
• Visits clients on a monthly basis
• Conducts mandatory screening when indicated to identify care needs.
• Documents all encounters according to organizational policies and procedures as directed by GRACE Program manager and gather and monitors outcomes and measurements.
• Creates survey measurements for GRACE Program outcomes and AmeriCorps deliverables.
• Participate in an AmeriCorps site health project. This includes community outreach, health interventions, and health events regarding, but not limited to, the following health topics: smoking cessation, diabetes education, nutrition, and healthy lifestyles, stress reduction, managing chronic conditions, and adolescent topics.
• Assist in collecting data monthly to evaluate quality and outcome metrics (to be determined). Reports monthly on number of clients served, referrals provided to clients, and client incarcerations/contacts with Emergency Medical Services, Emergency Room visits, and Hospitalizations.
