Senior Charity Care Foundation (SCCF) is looking for a few good people who want to give back to the greatest generation and obtain real-world experience through services to seniors. Members will represent the Senior Charity Care Foundation to ensure that they obtain critical healthcare needs.

Service Activities

Main activities include:
1) Providing access to affordable dental care/dentures through the coordination of the Mobile Dental
Clinics, and through the provision of eyeglasses, hearing aids, and other medical equipment not
covered by Medicare/Medicaid;
2) Maintaining and grow a network of healthcare providers that offer their services for free at a
discounted rate for qualifying seniors;
3) Providing preventative and diagnostic education to the senior residence staff and/or the seniors
including screening for oral cancer/stomatitis, diabetes, and hypertension.
SCCFs AmeriCorps member direct client service activities will focus on providing additional
support to our current program activities so that SCCF can expand its services, and provide outreach to both
seniors and the community in general. See Appendix H. for the Member Service Descriptions.

Direct client service activities include:
1) Hosting outreach events like ice cream socials to speak to seniors and care facility staff about the
services that are available.
2) Identifying which senior apartment complexes do not have service coordinators and go door-to-door to distribute information about available services;
3) Enroll and intake seniors by meeting with them to identify their needs, fill out the required
documentation for qualification, make referrals to services and resources as needed;
4) Coordinate the Mobile Dental Clinic services including appointment schedule, payment processing,
facility staff and dental team coordination and senior follow-up (which includes collecting senior
photos and stories to generate social media posts and the distribution and collection of the senior
satisfaction surveys.

Other member service activities include:
1) Building upon SCCFs existing network of healthcare providers by continuing to recruit doctors,
dentists, hygienists and other physicians that are willing to offer their services for free or at a
discounted rate for qualifying seniors;
2) Developing and delivering a presentation for various civic groups, government agencies and
nonprofits to generate awareness, develop partnerships and recruit volunteers;
3) Creating a procedure manual for the delivery and operation of the Mobile Dental Clinic services;
4) Recruiting and managing community volunteers.

Community Need Addressed

Affordable and local alternatives for senior medical services including dental and denture care, vision care and hearing care.

Position Outcomes

This work of this position will result in:
*an increased number of unduplicated seniors served per quarter.
*an increased number of visits (same senior, recurring services).
*a broader menu of services provided to include screenings for oral cancer/stomatitis, bleeding times, diabetes and hypertension.
*a network of healthcare providers increased from 45 to 60 professionals.
*an increased number of the recruited and trained community volunteers from 3 to 50.
*an increased number of residences served from 90 to 120.
