Position Summary:
For the Health and Prevention programs, the HealthCorps member will be responsible for recruiting, training, and providing technical assistance to all agencies and partners participating in CHP’s Health and Prevention programs. The member will support agencies in annual goal setting and assist in implementation of those goals by connecting agencies to local community partners and resources, national resources, and learning opportunities to support their goals. The member will also identify and coordinate hospital and healthcare-based safety net resources that increase access to care. Additionally, the member will lead on-site educational activities for parents, staff, and students when appropriate.

The HealthCorps member will also be supporting our Clinical-Community Connections work, through which they will be receiving comprehensive resource navigation training to support the community resource needs of identified CHP families. Upon completion of training, the HealthCorps member will be tasked with assessing at-risk communities for resources to support families’ health-related social needs, including resources related to housing, transportation, food access, childcare, career development, and more. The member will assist with disseminating new resources and policy updates for the Pittsburgh region to the hospital network. The member will also serve with families identified from the clinical setting, conducting needs assessments and providing personalized resources and referrals to key community partners, with close follow-up for ongoing support. Member will receive qualitative feedback from families to better understand how and whether referrals/resources were supportive in meeting their needs.

Major Duties and Responsibilities:
-Developing and providing health education for the community
-Gathering data on critical issues impacting the community
-Utilizing partnerships and data to coordinate referrals
-Providing education to clients and referring them to providers
-Providing health education to at-risk populations
-Building relationships with community partners
-Conducting research, mapping community assets, or reviewing data to strengthen the ability to meet community needs
-Developing and/or providing staff education or training
-Engaging stakeholders to understand goals, needs, priorities, challenges, and opportunities

Characteristics of an Ideal Candidate:
Self-starter, Multi-tasker; able to balance, Able to build strong relationships, Community engagement/outreach skills, Compassionate

Knowledge Required for the Position:
Knowledge of AmeriCorps/Health Corps member requirements, Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision:
The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

Review:
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.

Guidelines:
Written and oral guides provide specific instructions for doing service, Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects, Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations

Complexity:
Member has to develop, analyze, or evaluate information before the service position can progress, The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment

Special Considerations:
Unusual service hours, involving evenings, weekends, or long shifts, Health screening or service required

Criminal History Check Requirements Beyond NHC Standard Checks:
In addition to NHC Standard Checks, all NHC Pittsburgh members are required to complete and pass criminal history checks for Great Lakes Behavioral Research Institute/Diversified Care Management (DCM). Great Lakes/DCM is a third party vendor contracted with the Allegheny County Health Department (operating site for NHC Pittsburgh) to distribute NHC Pittsburgh member stipends. NHC Pittsburgh members are onboarded to Great Lakes/DCM system only when the checks have been completed and the member has passed those checks. Along with these checks, some Host Sites require their NHC Pittsburgh Members to be Vaccinated against COVID-19.

The checks that all NHC Pittsburgh members would be required to complete and pass for Great Lakes/DCM are the following: 1) Act 33 (Pennsylvania Child Abuse Clearance), 2) National Sex Offender Registry Clearance, 3) Act 34 (Pennsylvania Criminal History Clearance, and 4) Act 73 (FBI Criminal History Clearance - ONLY completed via Identogo PA).

NHC members do not have to pay for the completion of these checks. Great Lakes/DCM will distribute payment codes for the Act 33 (Pennsylvania Child Abuse Clearance) and Act 73 (FBI Criminal History Clearance which is conducted through Identogo PA ( PA FBI fingerprinting agency). The National Sex Offender Registry Clearance and Act 34 (Pennsylvania Criminal History Clearance) will be completed by Great Lakes/DCM upon receiving requested information that members will be asked to submit.
