Position Outcomes

The desired result: improve collaboration and effectiveness of diabetes self-management education services provided through local community clinics and two regional hospitals.
The new services developed by the PDV will increase access and participation in effective, evidence-based, diabetes self management education services for low-income and uninsured individuals who have diabetes or high risk of diabetes onset
