The Sanford Center for Aging's Health and Wellness Outreach Coordinator VISTA will build capacity for health and wellness programs designed for older adults throughout Washoe County, Nevada. Washoe County is one of the fastest ageing communities in the United States and 35% of older adults report having at least one disability. Low income elders face higher rates of limitations due to functional disabilities and are at an increased risk of being institutionalized. The Health and Wellness Outreach Coordinator will help reduce the impact of poverty on older adults through developing strategies to connect older adults with existing wellness programs while implementing collaborative approaches to health

Service Activities

The Health and Wellness Outreach Coordinator VISTA will:

Raise awareness of existing health and wellness programs through conducting presentations, meeting with community partners and senior housing authority representatives and social media.
Organize meetings for people living with chronic conditions and community partners to help identify existing barriers to participation in community wellness programs and determining what gaps in programming exist within our community.
Develop events and programs that address the health and wellness gaps that were addressed in the community convenings in collaboration with community partners.
Inform relevant community members and partners about the Sanford Center for Aging's Volunteer Mobility Pilot Program and our commitment to ensuring that all older adults who want to participate in community wellness programs can do so without concern of mobility barriers.

Position Outcomes
