Frontier Community Action Agency Client Outreach Assistant

The Client Outreach Assistant will assist in the daily functions providing needed services that meet the needs of low income
families and individuals.

Service Activities

Identify, outreach and recruit eligible program participants and volunteers, deliver local community presentations on
eligibility and program services, collect, organize and analyze information about program participants through records, tests, interviews
and professional sources, meet with clients to determine suitability for program participation and develop individualized case plans,
assist in the preparation and completion of reports as required by FCAA.
Active participation in the Family Stability Council, Community Breakfast and any other outreach activities, attend staff
and other meetings and training as required, respect confidentiality in discussing participant/consumer, staff, volunteers and
organizational matters.

Community Need Addressed

Disadvantaged individuals need assistance with health care, food stamps, housing, energy assistance and other resources available
to them in rural, northern Nevada. Many of these people are unaware of available resources, do not have Internet and some do not have
a place to live. Frontier Community Action Agency is a small nonprofit trying to provide assistance for a growing population of those in
need. It is important to FCAA to have AmeriCorps members to assist clients, recruit volunteers for programs, outreach to individuals
who need resources, and help distribute food to children and clients in need. All client contacts are logged in a database as are
referrals to resources provided. An exit interview/survey will be conducted to measure the level of healthcare well being following
member intervention and it is anticipated that clients who receive assistance will report greater well being in relation to their healthcare.

Position Outcomes

Increase in the number of seniors and disadvantaged individuals receiving resources and referrals.
