Being an AmeriCorps Health fellow involves providing health education, health screenings, and connecting patients to health care benefits and services. They will engage in community and organizational outreach efforts related to health education and access to health care services/benefits.

Service Activities

AmeriCorps Health Fellow activities will include:

Health Coaching – Provide health coaching in one-on-one or group settings.

Care Coordination – Provide health screening and connecting patients to health care benefits and services.

Case Management/Patient Navigation – Assist individuals with navigating health-related services/programs; provide eligibility assistance and enrollment in health insurance, coverage, and other health benefits programs.

Community Engagement & Outreach – Conduct outreach in the community (i.e. schools, community centers, events, and shelters) to provide education about available health services, resources, and related programs, and health care options.

Volunteer Recruitment/Management – Assist with recruitment, training, and retention of non-AmeriCorps volunteers; assist with the management of non-AmeriCorps volunteers.

Service Projects – Participate in various service projects; coordinate and engage in planning and participating in community service projects.

Training – Attend trainings on various health topics and resources as needed for placement, plus professional development training as available.

Position Outcomes

LifeLong Medical Care’s AmeriCorps program mission is to become a pipeline for careers in community health centers that are improving access to primary and preventative care services for the medically underserved communities that we serve. AmeriCorps Health Fellows would educate patients about disease prevention and management, and overall wellness that gives them the knowledge and skills they need to make smart choices about their health. They will provide and connect patients with health and social services in their communities that will help them lead healthier lives.
