Minnesota Community Care delivers clinical care, oral health, pharmacy services, health support services, research, teaching, health education, outreach, and social services for the people we touch. Our communities are diverse; their stories construct the spirit of our organization. The central goal of your VISTA project is to help expand the reach of DIVA Moms and ACT throughout the Twin Cities.

Service Activities

Build on the work of our 2020-21 VISTA in the area of Black Women’s health and pre and postnatal healthcare. Continue to strengthen our capacity in community engagement/outreach, communications, participant recruitment, and partnership development, as it relates to our DIVA program: Dynamic, Involved, Valued, African American Moms and ACT (Afrocentric Care Clinic and Team). DIVA Moms is an African-American-led program designed especially for pregnant and postpartum U.S.-born African American mothers and ACT extends safe space to all Black women across the diaspora.

Community Need Addressed

Inequitable healthcare systems especially in pre and postnatal healthcare for black women.

Position Outcomes

Strengthen capacity in community engagement/outreach, communications, participant recruitment, and partnership development, as it relates to the DIVA Moms program

Provide culturally reflective group prenatal care and resource networking to U.S.-born African-American moms.
