Healthy Futures Corps Member

Members work with health center staff to provide general health education, case management, and outreach services to patients in Federally Qualified Health Centers. We have sites across NYC and NYS. Members tend to focus on various health topics and patient populations depending on the health center location including but not limited to HEP C, HIV, LGBTQ Healthcare, Homeless health care, diabetes, nutrition, and more.

Service Activities

CHCANYS Healthy Futures Corps members make a difference by: Increasing Access: members provide patient navigation, case management, facilitated enrollment, and outreach services to medically under-served individuals and existing patients. Providing Health Education: members facilitate health education workshops; participate in health fairs and community events; and conduct one-on-one and group health education sessions with community residents and patients on healthy lifestyles and/or preventive health topics. AmeriCorps members do not provide health services related to prohibited items or activities as is outlined in detail during orientation.

Community Need Addressed

CHCANYS Healthy Futures Corps is a workforce development program dedicated to preparing its members for careers in community-based primary health care. Members perform a year of service at community health centers, where they work to improve and expand the delivery of existing services at their sites. Members perform direct service with patients thereby filling service gaps, increasing access to and improving utilization of health center’s services. The mission of CHCANYS Healthy Futures Corps is to assure the provision of culturally appropriate preventive and primary health care to medically under-served populations and communities. Health centers diagnose and treat budding health problems early, prevent unnecessary utilization of hospital emergency rooms and inpatient beds, and help cut health care costs. CHCANYS Healthy Futures Corps consists of about 15 members working full time in a Community Health Center in New York State. Each member works on a specific service project that reflect the needs of their health center, the objectives of CHCANYS Healthy Futures Corps and the member’s interests.

Position Outcomes

Members will learn to provide reliable, quality care to patients in Federally Qualified Health Centers across the city and state.
