Service Activities
Enroll consumers in health insurance plans, refer consumers and patients to other social service agencies in the community, implement evidence based programs to address identified health needs within the community, coordinate health coalition activities
Community Need Addressed
The state of Wisconsin’s uninsured rate is now at 7.3%. Unfortunately, 20% of Wisconsin Community Health Center patients remained uninsured in 2014, well above the state and national uninsured rates. This holds true in both urban and rural health center settings across Wisconsin. There is still a great need for connecting Wisconsinites with health coverage and providing them a pathway to access primary health care. In addition, we know that having health insurance does not necessarily equate to having a source of affordable health care. Consumers who have insurance for the first time often need support in connecting to and retaining their access to health care and many individuals often still need the sliding fee discounts offered by Community Health Centers in order to meet the cost-sharing requirements of their new plans. AmeriCorps Members will support consumers in connecting to care in appropriate settings.
Position Outcomes
AmeriCorps Members will be responsible for improving access to health care, connecting clients with resources to improve their health, and affecting modifiable health behaviors.
