Description

*JOB OVERVIEW:
The National Nurse-Led Care Consortium (NNCC) is in collaboration with PHMC Health Network and public health partnerships throughout Pennsylvania. The Public Health AmeriCorps Nursing Fellowship is designed to provide undergraduate nursing students with community based primary care and public health clinical experiences serving vulnerable communities. This unique fellowship will enhance students’ competencies to screen patients for Social Determinants of Health (SDOH), refer individuals to social and healthcare services, provide healthcare education to high-risk patients, and support the expansion of other capacity needs of primary care and public health organizations.
NNCC’s nurse training manager will mentor the Public Health AmeriCorps Nursing fellows and reinforce skills building and facilitate discussion based-learning throughout the service-term. Through community based experiential learning, the fellows will develop cultural humility and skills to enter the public health nursing workforce that will advance health equity, healthcare access and improve healthcare outcomes for vulnerable communities. AmeriCorps members will be a part of the new generation of public health nurse leaders who will advocate and advance community-based healthcare needs.
