Description

The Chronic Disease Management Prevention Capacity Building Project will partner with the PHMC’s Health Network (PHMC HN) and other partners, such as Einstein Health Network and Resources for Human Development Family Health Services to address the problem of uncontrolled diabetes and other chronic diseases among Philadelphia’s high risk populations. With the VISTA member’s support, programming throughout the PHMC Health Network and HPC’s partners will be strengthened by identifying efficient systems for recruiting and enrolling patients and new community members in the various self-management disease programming, which include community outreach and education. The VISTA will support systems that will permit the efficient continuation of participant engagement, recruitment and enrollment, as well as, follow-up to recruit lay leaders for the various programs.
