As we continue to identify ways to provide the highest quality of patient-centered health care to our community, NRHC is in continued need of a Care Coordinator to reach out to patients in need of preventive health services (such as cervical, breast and colon cancer screenings) and chronic care services (such as quarterly lab work for HIV+ and diabetic patients). This outreach will include notification of services due as well as patient education on the health and wellness reasons for which these screening and chronic services are recommended. A portion of the cancer related preventive services will include planning and helping run the Crucial Catch Cancer Prevention Awareness Day (community health fair) in October as part of our relationship with the National Football League/American Cancer Society’s commitment to increasing cancer screening. This role is currently fulfilled by our current NHC members and has been quite successful in engaging and educating patients in overdue care.
The Care Coordinator will also work closely with the site’s clinical Care Coordinator to outreach to patients who frequently access emergency rooms for non-urgent conditions. The Care Coordinator will work closely with these patients to invite them back into the health center for care instead of using emergency room services, and to understand greater barriers to care that prevented them from initially accessing the health center for treatment.
The Care Coordinator will educate and enroll patients in the online Patient Portal which will improve patient access to scheduling, test results, and direct communication with their providers.
The Care Coordinator will also participate in the planning and implementation of group visits for chronic conditions (such as hypertension, diabetes, and asthma) as well as preventive/routine care (such as pregnancy and well child visits). The Care Coordinator will have an opportunity to lead a portion of the group visits to promote patient education of topics pertinent to the visit. We hope to continue to expand our group visits. Expansion of these visits will require additional staff to help facilitate the visit while providers are addressing other components of the patient encounter. The Care Coordinator will be an integral part of this process.
There is no role at NRHC at present that currently addresses these needs other than the current NHC Care Coordinator. Without the Care Coordinator, we were previously limited in our ability to address our patients’ barriers to completion of needed services. We continue to develop and refine workflows that might eventually be taught to other volunteers, or potentially to staff once a funding stream is assured. At present, there is no standing source of funding for us to hire a full-time staff member in this capacity (nor has there been such a role in the past).
The member position activities follow the requirements related to supplementation, duplication or displacement of staff as outlined in 45 CFR §2540.100 (e) – (f). The members in this position will not be conducting AmeriCorps prohibited activities as outlined in 45 CFR §2520.65. The members in this position will receive no more training than 20 percent of the aggregate of the total member service hours as outlined in requirements of 45 CFR §2520.50. The member position description activities do not put members at risk for exceeding the limitations on allowable fundraising activity as outlined in requirements of 45 CFR §§ 2520.40-45.
