This position involves a combination of both Health Educator and Patient Navigator activities including providing health education, health screenings and connecting patients to health care benefits and services. Activities include: outreach and promotion of health education topics and healthcare services; scheduling and coordinating intervention/class logistics; facilitating interventions and classes about disease prevention and management and overall wellness; providing application assistance and enrolling patients in health insurance, Prescription Medical Assistance Programs (PMAP), and other health benefit programs; connecting patients with preventative, and primary and specialty healthcare services and resources; and conducting health screenings and tests.
