Description
The COVID-19 Responder/Wellness Screening Coordinator will, in addition to the above direct COVID-19 activities, work to address population health of patients including specific needs that arose or increased while primary focus has needed to be on slowing the spread of COVID-19. During the COVID-19 pandemic, rates for preventive screening measures have significantly declined as patients sheltered in place and visits were limited and shifted toward telehealth, in particular delaying routine mammograms, colonoscopies and pap smears. The COVID-19 Responder/Wellness Screening Coordinator will improve rates for cancer screenings of the cervix, colon and breast through direct outreach to patients to address overall care gaps, provide coaching/education and additional resources aligned with patient needs and safety, and help refer/schedule visits with clinic staff in person or by phone. The member will also improve processes through work with clinical staff, quality improvement teams, and subject matter experts to design new workflows compatible with telehealth that support timely completion of colorectal, breast and cervical cancer screenings and provide any necessary follow-up.
