Description
The COVID-19 Responder/Perinatal Coordinator will, in addition to the above direct COVID-19 activities, work to address population health of patients including specific needs that arose or increased while primary focus has needed to be on slowing the spread of COVID-19. The COVID-19 Responder/Perinatal Coordinator will focus on the unmet needs related to timely prenatal and postpartum care. The NHC member will work closely with the Primary Care Central Team and clinic management, including our OB Specialty Clinic, to centrally support patient outreach for routine prenatal and postpartum visits to provide coaching/education and linkage support for additional resources aligned with patient needs and safety, and help refer/schedule visits with clinic staff in person or by phone. The member will leverage EHR-based registries for Prenatal Care and OB deliveries to identify patients who have not attended prenatal/ postpartum appointments for follow up and improve processes through work with clinical staff and quality improvement teams.
