The Health Education & Outreach Member will be placed with the Department of Health and Human Services in Montrose County, CO. Responsibilities and activities of this position are to accommodate the high need for services and prevention education among all Montrose County families. We look for candidates that are passionate, intelligent, creative, self-motivated team players, and who feel strongly about the importance of community and individual health. We look for applicants with a background (preferred) or interest in public health, human services, hunger relief, housing, nutrition, and local food systems. Candidates should understand and be sensitive to the diversity among cultural, racial, economic, and other community demographics.

Service Activities

Duties include:
• Identifies target populations and analyzes the health education needs through surveys, statistic, and personal contact.
• Assists in coordinating and implementing opportunities with local partners to incorporate safety, prevention, and education activities.
• Prepares health education displays for internal and community use.
• Assists with prevention and behavioral health activities in the county.
• Assists with Montrose County 2019 Tobacco Community Profile and Implementation Plan.
• Assisting with health promotion efforts focused on healthy housing.
• Community outreach/engagement, organize events, develop promotional strategies and materials for print and social media.

Community Need Addressed

The Mountain Roots Healthy Futures Program builds community food security by creating synergies among education initiatives, nutrition and food access programs, public health and human services, and agricultural/conservation programs in the central Rocky Mountains and southwestern Colorado. Community-driven initiatives in the Healthy Futures Program include Farm to School, Sustainable Agriculture & Community Gardens, Hunger Solutions (Food Security), and Public Health programs, working together to build and sustain healthy communities, healthy people, and a healthy environment.

Position Outcomes

Members will serve on the frontlines for health education and outreach by assessing health needs of rural communities and assisting in the design, development, and implementation of regular health education, outreach, and promotional activities in line with the focus areas of Healthy Eating/Active Living, Behavioral Health, and Healthy Housing. Members may also work directly with priority populations, including people struggling to meet their basic needs, disproportionately affected by health concerns.
