Homeless Prevention Case Manager

The main purpose of the Crisis Prevention Case Manager is to provide families and individuals who are on the brink of homelessness with appropriate financial assistance towards past due rent, past due heating/electric bills, limited prescription assistance, gas vouchers for medical appointments, ID’s, driver license, and birth certificates. Crisis Prevention Case Manager also aide the grant writer with pertinent information needed to apply for grants to provide financial assistance. Crisis Prevention Case Managers provide resources directly to clients and do not provide referrals to other agencies.

Service Activities

Responsibilities include but are not limited to:
• Treat everyone who comes into the Crisis Prevention office with respect and understanding
• Supply clients with necessary resources and assistance
• Document and record provided assistance and client progress
• Determine client eligibility for utility assistance for San Luis Valley residents
• Network with utility vendors such as Xcel Energy Representatives, and maintain a good rapport with EOC contacts in Denver.
• Know organization requirements to meet the needs of clients who are requesting assistance from Friends of Man for dental, vision, hearing, and mobility needs
• Oversee and maintain Crisis Intervention funds to provide prescription assistance, medical gas vouchers, and identifications for clients
• Be knowledgeable of all grant information for each funding source
• Be knowledgeable of all paperwork necessary to complete rental assistance
• Complete all paperwork according to each individual grant guideline, and document all case management services provided to clients.
• Act as a supportive liaison for the other departments within Crisis Prevention as needed due to the variety of grants Crisis Prevention administrates
• Coordinate, advertise, and facilitate the School Back Pack Drive
• Coordinate and host the La Puente Community Christmas Party
• Participate in any Rural Alliance for Dignity related programming and/or events, including but not limited to orientation, retreats, evaluations, and trainings.

We have housing available for $400 a month. $40 of that goes towards a deposit that will be returned to you at the end of the term as long as the living area is cleaned and returned in the same condition that it was when you moved in. You will get your own bedroom and share the common areas with other AmeriCorps members.
