The Health Partnership Community Impact Coordinator VISTA

The VISTA will be part of the Community Impact team and will specifically work on the Yampa Valley Wellbeing Consortium, a 40+ member group that is facilitated by The Health Partnership to address shared population health issues across Moffat and Routt counties. The Community Impact Coordinator will support Consortium action team action plan virtual meetings; collect, update and communicate quantitative and qualitative data housed in the YVWC data dashboard; and contribute to social determinants of health training, education for partner organizations and community members.

Service Activities

The VISTA will support the Yampa Valley Wellbeing Consortium through communication and marketing efforts, gathering quantitative and qualitative data, and develop resources for the Consortium.
