Position: Health Educator
Reports to: Manager, Health Promotions
Collaborates with: Patient Programs Services Teams, Top Box Foods, Chicago Public Schools, Ann & Robert H. Lurie Children’s Hospital, Community Based Organizations

General Summary: The Health Educator is responsible for coordinating and facilitating health education programming and initiatives across multiple Erie Family Health Centers locations. The Health Educator will work with internal and external partners to increase access and education to our patients and community members.
Principal Duties and Responsibilities:
• Provide support to programs within the department
• Participate in and complete necessary trainings
• Evaluate department programming efforts
• Group Programming
o Groups Fitness Activities: Develop and facilitate bi-weekly or monthly group fitness activities for Erie patients
o Running Club: Promote program amongst Erie staff and community members; recruit Erie staff coaches to participate in programming; recruit patients to participate in weekly running club events; coordinate with Asthma Health Educator to identify and enroll participants into culmination event; lead and facilitate weekly running club activities
o Nutrition Education: Co-facilitate nutrition education and cooking demonstrations for Women, Infants, and Children (WIC) and case management programs at Erie clinics with WIC Nutritionist
o Buckle-Up Car Seat Program: Coordinate the Buckle-Up Car Seat Program at 2 Erie sites; complete 3-day Car Seat Technician training; collaborate with Maternal and Child Case Management to recruit patients; coordinate car seat deliveries and distributions with Lurie Children’s Hospital; educate patients on importance, proper use of car seats, injury prevention, and safety
o Breastfeeding Classes: Facilitate monthly breastfeeding classes to educate, encourage and empower moms
• Community Outreach
o Health Insurance Enrollment: Participate in and complete Federal Certified Application Counselor training; screen and assist in navigation duties to enroll patients in a health insurance plan; attend 11 insurance fairs
o Health Center Outreach: Promote the services of Erie Family Health Centers city-wide at 6 Health Fairs and other community events; provide information on access to care, health information, and targeted health education to community members on nutrition, physical activity, oral health, etc.; lead group-based nutrition education to community organizations
o Patient Outreach: Conduct outreach to Erie patients to assess patient social needs such as food, housing, and clothing, and connect patients to resources in their communities; distribute wellness packets to screened patients
o Top Box Foods: A community-based non-profit offering a variety of delicious and healthy boxes of food at affordable price. Member will coordinate the Top Box Foods program; collaborate with site teams to promote Top Box Foods to patients, staff, and the community; assist individuals in placing orders for Top Box Foods; coordinate with Top Box Foods team before and after deliveries; Lead and provide on-site support during Top Box Food deliveries; Ensure patient, staff, and community member retention; provide education and nutritional tips during monthly deliveries
• Other duties as assigned
